Synopsis: 106 patients underwent endoscopic thoracic sympathetic block (ETS) for treatment of underarm and palmar excessive sweating. Satisfaction rates were almost at 100%, but complication rates were also high.


As in the previous study, most patients were happy with the decrease in underarm and palmar sweating. However, the complication rate was disturbing. 17% had compensatory sweating, while 28% had gustatory sweating. Notice how the complication rate from this study differs from that of the previous study: A 28% rate of compensatory gustatory sweating in this study versus a 1% incidence in the previous study.


Conclusion: One conclusion that we can draw is that different studies yield different results for hyperhidrosis patients. The discrepancy of the complication rates between this study and the previous one was very large. Why? For one, techniques can differ among surgeons. Post-operative care may also vary for patients with excessive sweating. Sometimes the way a question is posed to a patient may alter his or her response. Both studies seem to support the use of ETS for treatment of excessive sweating in both the underarm and palmar regions. The complication rates, however, should be duly considered before undergoing any major surgical procedure, such as this one for hyperhidrosis.
